Formz has three plans: Starter (free), Pro, and Business. Pro and Business are billed monthly or annually (annual billing saves 20%) — see the pricing page for current rates.

Starter

3 active forms, 100 responses per month, all core question types, and CSV export — no credit card required.

Pro

Unlimited forms, 1,000 responses per month, file uploads, EVM wallet verification, OAuth identity blocks (Twitter, Discord, Google, and more), Brand Kit with branding removal, one-question-at-a-time mode, and email support.

Business

Everything in Pro, plus unlimited responses, 5 team members, multi-chain wallet verification and on-chain access gating, Viral Waitlist forms with points, conditional logic, webhooks, custom domains, advanced analytics, and priority support.

Upgrading or changing plans

Upgrade from inside the app any time a feature prompts you, or from your account's billing settings — changes take effect immediately and billing is prorated.
