Getting started with Formz
Create your first form in under 2 minutes.
Every form starts from your dashboard. Click Start a new formand you'll see a row of templates — Blank, Allowlist/Signup, Feedback Form, Application, Event Invite, Contact, Survey, Quiz, Web3 Allowlist, and DAO Contributor. Picking a template drops you straight into the builder with the relevant questions already in place; picking Blank first asks you to choose a form style.

Standard vs. Viral Waitlist
Choosing Blank opens a quick choice: Standard (a classic form, good for any use case) or Viral Waitlist (adds referral tracking, waitlist rank, and points for sharing — see Viral Waitlist setup). You can't switch a form between these two modes later, so pick the one that matches your goal up front.

In the builder
Once you're in the builder, use the canvas in the center to add and reorder questions, and the right panel to configure each question's settings, design, and access rules. Your work saves automatically as you go.
Publishing
When you're ready for responses, hit Go Live in the top bar, then use the Share button to grab your link, QR code, or embed snippet — see Sharing & publishing.
If you hit your plan's active-form limit, you'll see an upgrade prompt instead of a new form — check Billing & plans for what each tier includes.
