Identity verification

Formz can verify who's actually filling out your form, beyond a free-text name or email field. There are two families of identity blocks you can add from the builder's block picker:

OAuth identity blocks

Connect Twitter/X, Discord, Google, Telegram, or LinkedIn. The respondent clicks Connect, completes the provider's sign-in flow, and Formz records their verified username/email and profile data alongside their response — no password ever touches your form.

Wallet verification

The Connect Wallet block walks a respondent through three steps: connect their wallet, sign a verification message, and done. The signature proves they control the wallet address — Formz never requests a transaction or spending approval. EVM chains (Ethereum, Base, Polygon, Arbitrum, and others) are available on the Pro plan; Solana, Sui, Aptos, TON, Starknet, NEAR, and Cosmos are available on Business.

Gating access by on-chain holdings

Beyond just verifying a wallet, you can require respondents to hold a minimum token balance or NFT before they're allowed to submit. Set this up from the form's Access settings: add one or more lookups (balance, token, or NFT checks), then choose whether any or allof them must pass. Respondents see exactly which conditions they meet and which they don't before they can continue.
